do not sign ahead of time.
Vaccination records - Bring all previous immunization records.
Self-Pay - flat rate $600

Things done in Clinic
Bloodwork
- Quantiferon - QFT/TB (everyone over age 2)
- Syphilis (everyone age 18-44)
Urine
- Gonorrhea (everyone age 18-24)
Chest X-ray (only if TB/QFT is positive)
Immunizations you need can be done through the Health Department or any local pharmacy like Walgreens, CVS, etc…
Immunizations done elsewhere are not included in the payment.
Physical Paperwork
Once all vaccines/vaccine records, bloodwork results, and/or X-ray report (if needed) are received
- Physical form will be sealed in an envelope (DO NOT OPEN)
- A copy of the physical for your records is included
You will receive a call from MyCare/Dr. Naseer when paperwork is ready for pickup.
- Usually within the same week, once everything is received.
